Visit Reason
The inspection was conducted as an initial licensure survey for The Pointe at Meridian healthcare facility.
Findings
The facility was found to have multiple deficiencies including incomplete criminal history and background checks for employees, lack of effective corrective actions for resident incidents, late completion of admission agreements, improper medication refrigerator temperature maintenance, incomplete negotiated service agreements, insufficient personnel certifications, and lack of documented staff orientation.

Three of three employees did not have a Department Criminal History and Background Check completed.
Two of two employees did not have an Idaho State Police Background check completed.
Facility did not ensure effective corrective action to prevent recurrence for Residents #3, #5, and #6 after incidents.
Six of seven residents' admission agreements were not created and signed on or before change of ownership.
Medication refrigerator temperatures were not maintained between 38 and 45 degrees F, ranging between 30 and 37 degrees, with no corrective actions taken.
Negotiated Service Agreements were not completed within 14 days of admission for Residents #1, #3, #5, #6, and #7 after change of ownership.
Three of seven staff members lacked evidence of CPR or First Aid certification.
Seven of seven staff records did not contain documentation of 16 hours of orientation.
