The inspection was conducted following a complaint investigation into a reported verbal altercation between an employee and a resident at The Grace Home 2.

Complaint Details
The complaint investigation was triggered by a report of an employee to resident verbal altercation on 12/26/2024. The facility reported the incident to Adult Protective Services on 12/28/2024 but failed to report directly to the Division of Licensing and Certification. The verbal abuse was confirmed by video surveillance and progress notes.
Findings
The investigation confirmed that Employee #1 verbally abused Resident #1 on 2/21/2025, including threats and refusal to assist with a medical need. The facility terminated the employee involved. Additionally, the facility failed to immediately report the resident rights violation to the Department of Health and Human Services as required.

Specifically, Resident #2's MAR lacked a required signature on the back, and expired standing orders for Residents #1 and #2 were still being transcribed.
