The facility failed to ensure that only ambulatory residents capable of self-preservation with minimal assistance were retained, as evidenced by two residents needing total assistance. Additionally, the facility failed to utilize appropriate safety devices to protect residents at risk of eloping, with no safety devices or alarms on exterior doors.
