Visit Reason
Facility failed to ensure resident move-in evaluations and service plans addressed all required elements and were reflective of resident needs. Psychotropic medication use lacked resident-specific parameters and documentation of non-pharmacological interventions. Acuity Based Staffing Tool was not completed or updated as required. Interior materials and surfaces were not kept clean and in good repair. Residents lacked individual privacy and keys to their units. Multiple other deficiencies noted.
