The re-licensure survey identified 11 deficiencies including failure to promptly investigate and report abuse, incomplete resident evaluations, inadequate service planning, failure to monitor fall interventions, inaccurate staffing plans, incomplete staff training, lack of resident keys, administration compliance issues, and restricted access to secured outdoor areas. Many deficiencies were corrected by the revisit on 2024-02-07.
