The inspection was conducted as a State Licensure Change of Ownership and Change of Category survey for a Residential Facility for Groups, to evaluate compliance with Nevada Administrative Code Chapter 449.

Findings
The facility was found to have deficiencies related to kitchen sanitation and first aid/CPR training. Specifically, the kitchen sink and counter were not clean and well-maintained, and one employee had not completed required First Aid training.

Deficiencies (2)
The kitchen sink had a build-up of black material around the rim and two counter tiles had open cracks; the sink and tiles needed repair or replacement.
One of eight employees failed to complete First Aid training as required within 30 days of employment.
